Forget me Not

FMN (Forget Me Not Pet Cremation) provides pet cremation services to both Retail and Commercial customers. The clinic-portal is a submission portal for FMN's commercial customers. Once a contract is established Clinics, Animal Hospitals, and Emergency Centers are provisioned a company and user account in the FMN portal and can begin submitting cremation requests. Once the request is submitted FMN administrators are alerted via email and SMS. Then FMN administrators use internal off-line processes to pickup and process the remains. FMN Administrators then update the status of the cremation form.
